There's an expansion to Afterbirth's greed mode. There are two new characters added, one of which is quite challenging to unlock. There are better tools for modders, and through the workshop it's very easy to install mods. Through the in-game menu, you can choose which mods you want to have on. After beating mom's heart, mods no longer disable achievements. There are tons of new rooms, enemies, and even a few bosses. That's plenty to make it worth the $10..
